If caught early you can get by with changing just the damaged piston. Am old time engine expert can tell you if the sound is camshaft speed or crankshaft speed. Yes that happens but still, the OPDA issue is real enough that a couple aftermarket companies like Crown came out with "fixes" for the OPDA problem that only happens in 05/06 models. They're not really an actual fix for the root problem caused by a redesign of the OPDA, more like a band-aid that helps.
